Download all license applications to your computer before completing. Some applications are fillable and can be typed directly into, then saved and printed out. Others you will need to print out and fill out by hand. After you have completed all your applications, make sure to sign, date and submit with payments to the City of Port Washington by Thursday, April 15, 2021.

Applications and fees can be submitted by mail, placed in our new Drop Off Box inside the front doors of City Hall in the atrium, or dropped off at the City Clerk's Office at City Hall, Monday - Friday, 7:30 a.m. - 4:30 p.m. If you have any questions please contact Susan Westerbeke, City Clerk at 284-5585, or by email.

Alcohol License Renewal Submission

Use Original Alcohol Application if any changes have occurred to your business or agent.

Must include:

1. Renewal Alcohol Applications
2. Copy of current WI Sellers Permit

Additional License Application Submission

Please download, complete and submit along with your Alcohol License Application.

May include:

• Annual Cabaret License
• Cigarette & Tobacco Product License
• Amusement License
